New Patient Registration
Patients living within a 7.5 mile radius (5 miles out towards the Lisburn area) of the surgery can register with the practice. Please contact the surgery on the usual number if you wish to become a patient.
Glenavy Family Practice does not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
You will be required to bring your medical card, proof of identification and a utility bill when applying to register with the practice. If you do not have your medical card you can completed the attached application form.
If you are registering from outside the UK
In order to obtain a medical card you need to fill in an application form, which is called HS22X. This form is available in English and in a range of other languages. Please click on the respective links below to download the form in English and/or in translation.
All application forms are available at the practice
Patient registrations will be dealt with once a week. New applicants will be offered a pre-registration medical. Details of all current medication will be required when attending pre-registration appointment.
